Well here are the sleeves if u still want them , these are those i've ported ... enjoy !

How they look in 3ds max , well u need 3ds max first to open the file ( 2011 or 2012 version )

PS : well i dont recommend using milkshape , they will look very ugly ( since milky doesnt have the choice for smoothing them quite well ) , try to use more 3ds max or my favourite cinema 4d
